I have the hand strap and I still don't know if I have it on right, but it works.
BTW: I presume you have BG-ED3 on camera, it will not work without it (or so I remember).
Don't have picture handy since I am at work, but what worked for me is put the hand strap Canon logo out with C in Canon at top of camera, then connect like you do the Neck strap. Except the one strap went all the way through the top strap loop on the camera and then connected to hand strap at bottom. So both ends of the strap are on the bottom of the hand strap.
Hope that helps until I (or somebody) can post picture.
It would be really handy if they included instructions with the handstrap.

I got a hint from:
http://www.dpreview.com/reviews/canoneos1ds/page3.asp
--He placed the metal mount (with three threads) on the top of the camera so you can still attach the shoulder strap.
My feeling is that the metal mount will scratch up the 10D's casing, so I put the plastic mount on top intstead.
Either I don't use the shoulder strap now that I have the hand strap
