I found some model releases that fit my criteria... Are there restrictions on just copying it and changing the name???

Here try mine
I ..........................................(print) do hereby state that I am over 18 (eighteen) years of age on the date that this release is being signed and that I was over 18 (eighteen) years of age on the date that the photographs* were taken.
*Photographs encompasses, still photography (digital electronic photographs and/or printed photographs), and /or video tape/film photography or any media deemed to be photography. The undersigned waives any and all rights to said photographs and negatives and/or video tapes/film as covered by this release.
The undersigned will receive monetary compensation in the amount of $.................for his/her time and photographs, and if agreed to, by the photographer copies of some of the photographs covered by this release.
I shall not expect nor will I receive compensation for any sanctioned** use of the said photography by Kennedy Management Inc., their agents and/or their representatives as agreed to within this release.
**Sanctioned use meaning print media, advertising, merchandise, and/or promotional material, electronic retrieval systems (the internet / world wide web) and/or any media which Kennedy Management Inc., its representatives, employees and their agents see appropriate, for use in all markets domestic and foreign.
I give Kennedy Management Inc. total permission to use any and/or all, in full and/or part, altered or unaltered, photographs and/or video covered under this agreement for any sanctioned use from this date forward.
I declare that the material that is being submitted to Kennedy Management Inc. or the work I have undertaken is not in conflict with any other contractual agreements with any third party.
I declare that each section of this model release was explained to myself and that I was given opportunity to questing any section of this agreement before I signed it.
I declare that this statement has been made freely and without distress. Further, I agree to indemnify, hold harmless and defend Kennedy Management Inc., it’s associates, employees, agents and representatives harmless against any and all litigation or other legal action taken as a result of the use of the material accompanying this document.

Depends on where you live & your State laws. Title it "Photographic Agreement" instead of LEGAL RELEASE OF ALL YOUR WORLDLY RIGHTS, too. :D

Miller, mabye you should be a comedian not a photographer! Im jokin, thanks for the help.
I was thinking of going to a laywer in my area and having him make one for me based on criteria I give him... This way im following all local laws and current information. What do you think?

Unless you're really concerned about this, I wouldn't spend a penny on a lawyer. There are plenty of examples out there. Pick one, modify it and use it. Seriously.

Everyone is free to use mine, I got it from someone who said I could use it free and then modified as needed over time. It looks pretty close to a lot of others.

I do recommend, of course, that you change the names on it otherwise all copyright reverts to me. :evil: :) :) 8)
Almost forgot, I did have a lawyer look at it at one point and she said it would work as good as anything will - meaning nothing is gauranteed.
