Can someone point me in the right direction for a model release form that would have tick boxes for the type of modeling they are willing to do.

I saw a pdf ages ago but never saved it and now cannot find it :(

I have to express my curiousity (and maybe ignorance).

Why would you need the release to have a check box?

I would think that before you get to the sign the release stage you already know what a model is willing to do or not. I can maybe see if you were going to shoot the model over a period of days or months so you can remember what you can ask them to do, but for most one day sessions, it should not be that hard to remember their limits for that shoot.

It sounds more like you need an model agent or model representation agreement form, which would be more appropriate for indicating what type of modelings they are willing to do.

To make your own I suggest looking at what all categories are available on www.onemodelplace.com for models and using that for the check boxes and then just add to an existing model release.

Just curious if I am missing something,

The check boxes Would be used for the model to indicate what type of photography she feels ok doing.

But you should already know that (or at least I always do) before the model even comes over. It would be too late in my opinion at the model release signing stage.

Not making sense to me.

also, if you shoot many models a week some pro some not it just helps in thought process as well as not offending you give the model the option.

The one I use states has a big paragraph stating that the photo session was conducted in a professional manor, the model participated in all poses willing, and was not coierced [sic] or forced to pose anything against their will.

So yeah it does leave me open to them regretting doing a pose or not happy with the results after the fact and not being able to use any of the stuff; and me ending up wasting my time.

So does having a clause like this open me up to liability by having them sign after the fact? Should I be having them sign a release before hand, and then a professional conduct form after the fact? (Total CYA'n here)
