Is there a case, as little slop as possible that will house the G1 with Lensmate adaptor and UV filter...
and possibly include the 420ex Flash as well, and still get to buttons, etc.

I agree with John! I have had Lowepro Nova Micro for a while (available in several colours), it carries easily my G1, 420EX, Lensmate, remote, and 5 filters and lenses (all in separate plastic cases).
Well, if I have the flash with me I cannot fit the camera in Lensmate attached, but that's minor limitation to me.
Price: Below $30.

It looks like you want something to "clothe" your system while you use it. In all the forum activity here and at DPReview, I don't ever recall seeing one mentioned. I just bought the bracket with neck strap from Photosolve (http://www.photosolve.com). Sturdy enough to hold a medium-sized flash (you need to buy the flash bracket accessory) and keeps the camera in a good chest-high position. Look for the Extend-a-Bracket. They also sell the Extend-a-View Pro loupe and shade for close focusing on the lcd screen. I just received mine, so have not tried it on a macro shot yet, but it works.
