Was the effect done by using the shadow/highlight feature?

No. It was done only on site, using flash...

Flash hand held and zoomed out to create a spotlight? I may be wrong, but it looks that way. It is a way cool effect that I use at all of my dance and prom shoots, it really draws the eye to the important part of the image.

No. The flash was onto the camera, but it was tilted at 90 degrees and bounced in his white cardboard.
