Quite easy to do with Photoshop -- many different ways, as well. Probably easiest would be to use the dodge tool. Select the appropriate sized brush, select midtones and adjust exposure setting to 50%. Dodge incrementally, dragging over a small image at a time. You may need to magnify the image. Continue dodging until you reach the desired look. Do not dodge too much or it will look quite unnatural. The reason to do it incrementally is so that you can go back a few steps if you lose the texture of the teeth or it begins to look unnatural.
For the eyes, I'd capture the white parts of the eyes, then put them on their own layer so that I could dodge easier without affecting the other nearby areas.

I like to select the desired area, then feather it 2 or more pixels then adjust color/saturation/contrast.

i think you should go for the grittier look, and stay with the rotting teeth.

I find it's best to go light on whitening teeth and eyes. Just a tiny little too much and it looks phony and sticks out like a sore thumb. You can dodge 'em, but then use a light, light blur brush so they don't stand out too sharp, or in the case of eyes, too glittery. Very tricky 'cause eyes and teeth are the two things we look at first.

There is whitening and then there is bizzro world teeth. If you are working with real people and just want to improve the look then one of the best ways is to make a selection of the teeth,(not the gums just the teeth) place a light feather to the selection and make an adjustment layer (Layer/Adjustment Layer/Hue and Saturation) you can link this to just the background layer. Or if you want copy this selection to a new layer you can then do the adjustment layer to just it.
With the Hue and Saturation menu open first pull back the Saturation back until the yellow is almost gone. Then pull the Saturation to add some cyan which will neutralize the yellow but not to the point of pure white. If the teeth are pure white they will not look natural. The same can be done with the whites of the eyes since most of the time for eyes it is red that keeps them from white.

For quick touch ups, I just use the sponge tool set to mode=desaturation and flow=50% and use a soft brush. Most teeth aren't pearly white, so I try not to remove all color. Works for me. :D
