Do you use any softboxes, reflectors etc, I am looking for a more reliable way then bouncing off walls and ceilings. Thanks

The only "accessory" I use often is a white handkerchief or paper to cut down or diffuse the intensity of the flash when bounce isn't an option.

Stofen Omnibounce for the 420EX. You can get it from B&H.

Omnibounce here as well, but for the 550EX. Only thing I add to the flash and then not very often.

The best way to use flash, is off the camera on a flash bracket attached to the camera with the Canon Off The Shoe Cord 2. The reason for that is at least threefold:
1. By increasing the angle between the flash and the subject you greatly reduce the chance of "red eye", the light reflected off the subject's retina or vascular back of the eye.
2. By moving the flash up and to the side, you create more depth to the picture, rather then the "headlights in the eyes" look.
3. Various flash brackets allow you to rotate the camera or the flash so that you have the flash in proper orientation for portrait shots.
