When shooting full manual, just wonderinf if the onboard flash output is adjusted automatically by the camera depending on the subject?

Yes it does. Just think for a second about how different your flash pictures would look if the flash power remained the same while your aperture changed from f/2.8 to f/8. For Canon this adjustment method is called E-TTL, currently on its second generation E-TTL II (E-TTL = evaluative through the lens)

Assuming you're referring to Canon digital SLRs,
The built-in flash uses E-TTL flash metering. No manual flash possible.
But it's still a good idea to use Manual mode on the camera when using the flash.
Adjust flash exposure with FEC.
