'No-name, common ordinary budget bracket' found in retail outlets all over the place. Note that this type of bracket is primarily useful for a photojournalist who turns the camera to portrait orientation to take a news shot, as the flash sits almost directly over the lens in this orientation, so that shadows are minimized (good!). But in the normal landscape orientation of the camera, this bracket only serves to MAKE shadows OBVIOUS (bad!)
Also, with this type of bracket, any flash which is dedicated for use with your camera must be used with an additional expensive cord (from Canon) or you lose the flash dedication...a PC cord merely triggers a flash, but maintains no 'smart connections' between camera and flash.
In the case of the above flash and bracket, they are sold as a unit. (You only need to Google 'DS328AZ' to find it!)... from this website
http://www.camerahouse.com.au/products/ProductPreview.aspx?productID=1026&productName=DS328AZ%20Digital%20Flash
:
"DS328AZ Digital Flash
This electronic flash is designed for use with any digital camera with a built in flash by means of the included bracket and with any digital camera with a hot shoe. The flash can also be used ‘off’ the camera as a remote wireless flash. The distance chart combines with the zoom head to set the correct shooting distance for up to 3x zoom lenses. Features include 330° swivel and 90° tilt head, mode selector switch, ready indicator and flash test button. Flash sync cable included GN28 (ISO100/metres), requires 4 x AA (alkaline or Ni-MH) batteries."
What it does not make clear is "This flash works in a dedicated hot shoe, but not while on the supplied bracket, and not with ETTL compability" and "This bracket allows the flash to be used on any digital camera in Auto mode (not ETTL compatible) if you have a PC cord connector on the camera (otherwise you have to buy an adapter)." In brief, stay away from the flash unit, and reread again what I wrote about this type of bracket (above).

a 'flat flash bracket'...that puts the flash next to the camera. Try a bit of searching at on line stores on the web! The first one I visited was Adorama and found these...
http://www.adorama.com/MZ54036.html?searchinfo=bracket&item_no=147
http://www.adorama.com/INKSK9.html?searchinfo=bracket&item_no=215
http://www.adorama.com/FABS.html?searchinfo=bracket&item_no=289
And there are other stores that specialize in a lot of accessory items, like Porter's. Google them.

20dshooter,
Keep in mind that the Canon Off Camera Shoe Cord 2 has a 1/4-20 threaded hole in the flash end for attachment to a bracket. A device like this could be made with an appropriately sized piece of aluminum, few screws and rubber washers.
Just something to consider if you have the inclination. On the other hand, a bracket like this shouldn't cost much. This sort of stuff is typically sold in local camera shops.
