Does anyone have a copyright release I can have to send to clients so they can print from image DVDs I give them?

What you want is a print release, not a copyright release. Two different animals.
A copyright release would allow your clients to do whatever they wanted to with the images, like sell or give them to other vendors or publications.
A print release only gives them permission to make prints from the files, usually only up to a certain size if you're looking to make money from larger print sales.
It's easy to write up your own. Could be as simple as...
John and Becky Smith have permission to use the images contained on this disc for (what you want to limit them to).
Signed (your company logo and digitized signature).

Good info. But depending on where they are getting it printed, some placed don't even ask for that. The last wedding I did, I gave the photos in a DVD to the B&G and they copied and uploaded onto an online printing place like yahoo or something like that and still got it printed, even though i tagged it and copyrighted. Of course they had my consent before doing so, but just saying that some places don't even check.

I use lightscribe and put my print release on the dvd. You can also do it on the dvd cover.
