Neat Image and Noise Ninja are the best tools.

I use neatimage as a plugin for photoshop...it is great. It is also very easy to use. As a result, I am not afraid to shoot in ISO 800 anymore. I wish Adobe would license this technology and bundle it in as a standard plug-in.

Here is a sample of what Neat Image can do. I just posted this same thing in a different thread, but I find it proves the point well. BTW, it's a screen capture of Neat Image in the preview stage of processing.

I find that if you can, slightly over expose images when shooting at a higher iso. Or, atleast make sure it is properly exposed. If underexposed, and you have to compensate in software, you will end up with alot of noise.
As far as software goes, Neat Image is great. I used it for about 6 months before discovering Noise Ninja. I like Noise Ninja even better. Slightly better results than Neat Image, and it is faster.
