I suppose in your administration pages, you choose the language you want to setup from the pull down menu.

Am I right?

When language is set "lazy" it means only UI stuff is translated (technically speaking: only translation files are used) and content is taken from primary language. When language is NOT lazy every item in editors marked "multilingual" is used as content. Change editor language means changing language you currently edit.

Thanks! If I set language NOT lazy how will it handle IPTC subcategories ? Does it automatically assigns existing EE categories in every language according to category numbers if the IPTC sub categories are only in German or do I have to assign the categories for each language manually?

You have to think category names as "virtual". When category is assigned to photo, it is not assigned a category name but an id in database, which is linked to category name in all languages. You can edit the names and all language variations of it in category editor: you don't have to reassign any category - the names change as you save category editor.
