Looks like his skin bunched up and shadow noise makes it look weirder than it does in real life.

Hmmm, seems like its something on the child's clothing rather than photo-based.
Send it to the FBI, you'll have an answer in two to three weeks :D

I agree... I imagine this was shot at a fairly high ISO... so when the earlobe fell into shadows it exhibited some funky shadow noise.

Either that or you've got a piece of HUGE dust on your sensor
