Hi,
I have a Manfrotto 190B monopod and 352RC ball head (with quick release) and I've used that combo with a rented 300L f/2.8 IS without any issues. Not sure if that would work with a heavier lens/body combo though.
Cheers!

I have 400 2.8, thanks to the advice from folks here I got a wimberley plate and clamp head for my manfrotto monopod. It is heavy duty and I have no worries about some sort of accident happening involving the head.

Manfrotto- 680B (3249B) Compact Monopod (Black)
Supports 22lbs, I think it's in the $50 range. I have nothing bad to say about it, other than the clamp tightener tool was lost within 2 days.

i use a manfrotto 681B on my 300. works great. head wise, i rarely use a head with it, generally just put it strait on the lens, but the 488RC2 works well. I really dont see any advantage to putting a gimbal on a monopod, so i would say no to that.

Gitzo 1558 has a built in pivot, so it does not need a head. Have used it with the 400 2.8 also.

Whoops!
I mentioned the Manfrotto 190B... heh ... silly me, that's my tripod! :o
My monopod is the Manfrotto 479B - now replaced by the 679B. Nice and light, yet sturdy enough and can take a good beating.

You need a gimbal head for the 400 on a tripod. On a ballhead it will not be steady. Just get the Wimberley gimbal head and don't waste your money on a ballhead like I did.
I use:
- G-1588 Mountaineer Mk2 Tele Studex Carbon Fiber Monopod (without any head).
- G-1227 Mountaineer Reporter Mk2 Carbon Fiber Tripod (with wimberley head).
