Order Forms for Pictures?

I have my first league photoshoot coming up and I was wondering where to go to order the "order form envelopes"? Do you guys have a place online where you can design and order? Do you create a template and take it to a local printer for them to print?

Or do many of you have a order form sheet and have the parents staple a check to it?

You might try ULINE (http://www.uline.com/) in Atlanta and see if they've got anything you could use ....

Other than them, it might require something "custom" ....
