Order Forms for photo session

I had posted this question in another section of the forum but thought it may be more appropriate here.

I am doing my first league shoot next month and need order forms for the parents. I was wondering where some of you order your envelope forms from? Do you create a template and then take to a local printer? Is there an online business in which you can create a template there and have them print? Any feedback is appreciated.

I do mine on business envelopes, just print them myself on front/back with packages and then individual items. You can have them printed for you, but it's cheaper to do it on your own of course.

I've looked for blank business envelopes that I could print my own info on through my color laser but can't find a vendor that sells them...I was thinking of a 8.5 x 14 legal size with a flap that could be sealed with $$ inside....does anyone know of where I can buy these?
