1/4-20 generally, sometimes 3/8-16. The bottom of a camera (P&S, 35mm, DSLR) is almost always 1/4-20 so you can judge by that.

I think there are two sizes, 1/4" and 3/8". Sometimes there is an 3/8" adaptor that can be removed leaving the 1/4" screw. Some tripods are sold without a head and the head screw mount is 3/8". Most cameras have a 1/4" thread mount. This is off the top of my head. Somebody can correct me if I'm wrong.

Yep. My monopod has a 3/8-16 screw, which pushes down on a spring to reveal a 1/4-20. My ball-head has both. I think medium format and/or large format cameras take 3/8-16.

Doesn't matter. It seems you're right anyway. :)
