Pricing of book jacket...design and photos?

I have been doing my research on the topic and after reading numerous threads and looking at pricing calculators I am still unsure on how much to charge. Here is what everything will consist of:

Book of Quotations
200 print first run, author just wants to break even
Photography and graphic design on cover
Head shot on back
I estimate it will take around 8 hours for shooting, design, retouch and licensing

I couldn't find anything for such a small print run, but for a larger run (<10,000) it looks like $1500 would be the average for the full cover shot and head shot on back alone.

I have also read about pricing by the hour, which is making me lean towards about $100 an hour. Any input would be appreciated on how to go about pricing this. It is his first book and doesn't really know much about the pricing either.

I would charge $95-$150 an hour, no need to complcate things.

It's effectively a commission rate - I'd probably look for a day rate of $800-$900 plus expenses / tax etc...

You could do a percentage of the price of the larger volume of prints, since you know what a larger publishing would be. You could include a provision for future payment in the event of reprints. Or you could go by the hour.
