Moving 1.22 install to 1.5 on another server

I've got a successful EE 1.22 install on one web host's server. I am moving the entire site to another web host and want to move EE with it, except I'd like to move it into EE 1.5rc4.

Assuming I get a clean, successful install of EE 1.5 on the new server, what would be a recommended way to move the database from 1.22 to 1.5, and the photo/folder structure from the old to the new?

Failing that, are there upgrade instructions for taking EE from 1.22 to 1.5?
