I found this shot interesting.
What mistakes were made and how could I improve this shot.

IMHO, the shot has potential, but here are opportunities for improvement:
First thing that stands out to me is that although the Fall leaves are colorful, they are rotting, discolored. If you are attempring a shot of Fall colored leaves, at least find some that are healthy & purtty!
Next is the composition- don't like the way the leaf is cut off at the front and the DOF seems poorly placed - blurry so close to the front.
The lighting is kind of flat. Punchung up the contrast would help this.
Keep shooting and looking for GOOD specimens!

either frame tighter ie. concentrate on just one part of the leaf to fill the frame , or use a smaller aperture to increase depth of field.
also try to get the angle of the leaves so they are more interesting in relation to the frame , (e.g. second version below) crop tighter etc.. this way you can use the depth of field to lead the eye to the nicest part of the leaf which is placed (now) nearer the apex of one of the thirds of the image ( search for 'rule of thirds') e.g. see below

I would suggest putting more thought into depth of field and point of focus. It is difficult to decide what to look at in this picture because of the confusion of in focus and out of focus material. Sometimes you have to find a different vantage point and perspective in order to get what you want. Also, it is often the case that you want virtually everything in focus, in which case it is necessary to stop down all the way and set up the focus at the hyperfocal distance.

I would try finding some that you can use the sun to back light the leaves, it really brings out the detail, here is a thread that I posted a couple of my shots of leaves.
