It is different depending on where you are based (Canada, USA, Australia, Europe). Aus seems pretty easy if you are a working pro. For Aus. you appear to need 2 pro bodies as well. Canada it isn't necessary, but they do require at least one pro body, and track your SNs.

- Six recent examples of your published work (including photo credit),
or six recent examples from your portfolio (either hard copy or digital
format). Compulsory for all applicants.

- Samples of business stationery (business card, letterhead),
advertising material or certificate of business registration.
Compulsory for freelance applicants only

- Membership to a recognised professional photography organisation
(if applicable)

- Reference from employer, Editor or Photographic Manager.
Compulsory for corporate and staff photographers only

- Two passport sized photographs of the applicant, either hard copies
or an equivalent resolution JPEG file for the CPS membership card

When we got our membership there, there was no need for anything special in terms of prints. We just got a couple of our random shots developed at the local Costco (pictures of my cat actually) and sent that in. They did not say anything about the shots not being enough.

We also provided a list of equipment, and receipts for a couple of hundred thousand worth of Canon equipment. So that might have helped. :)
