I have one for sale. I purchased it last year and it has a date code of UV0311. I am selling it for $890 w/ free shipping but you have to pay the Paypal fees. It comes with a Kenko lens protector (not UV filter), hood, pouch, and manual. But I really do not know what I did with the box. Will be packaged seriously for shipping.

I will post some pictures in the morning. I do not have my camera to take a picture right now.
