I am using a portaflash set with my canon on manual and I am getting these images, it's important that I get this sorted out I can't afford to waste time!!

It would apear as if your flash and your camera are not synced.
What is the shutter speed on that shot?

I'm not sure about that one but as soon as I get to somewhere where it looks properly exposed (around 500 f22) it does this
and not on the over or underexposed settings

No chance it will sync at 1/500th! try 1/125, you may get away with 1/250 (I can on the MkII and 10D)

This is the first time I have used flash with a digital slr , I have absolutely no idea what I am doing, to tell you the truth I have only probably used flash a couple of times, I don't get it :) I am doing macro shots too,

That is caused by the cameras inability to sync at the shutter speed your using and the flashes inability to high-speed sync with the camera at that shutter speed.
The rebels max sync speed is 1/200 (so start there). If your using a tripod i cant imagine why you'd need to go much over that (in fact you should be able to get away with much less).

This is a "shot in the dark" but depending on how you are firing your Portaflash set off, it is firing too early. I have seen that with the on-camera flash it will prefire to get flash exposure. If the Portaflash is working as an optical slave, this will give the image you are getting.
However, I do not know how you are firing the portaflash set off.
Give some more detail as to your setup and how you are firing off.
