Cute kid. There are several problems with this photo. You say you "shot for his focus" I am guessing that you mean you tried to put the focus point on the baby. I think you missed the focus though. Seems the focus is on the fingers that are sticking up. So the face is out of focus. With an aperture of f/4 and a focal length of 68mm and a distance of 10 feet (I am guessing on that) - you only have a 1 foot depth of field. Try bumping up the ISO and stopping down to get a larger depth of field. Since you are using flash, there shouldn't be a problem with exposure. Try not using a flash and just make sure you have a fast enough shutter speed. That will avoid the harsh light. Also watch out for objects in the frame - like whatever that dark part is in the bottom right.

Some more advice, but first, welcome to the POTN. I'm sure you will get an education in photography here in Critique Corner.
It is of little purpose to post full images for which you are expecting critical reviews of focus or sharpness. The problem is that by the time you have downsized the image and compressed it in JPG, the detail the viewer would need is gone.
We suggest that you pick a small area of your image that you want to present to us and post that from the original as a full detail, 100% crop.

A more controlled way of posting a 100% crop -
Select the marquee tool - then in the options palette select Style->Fixed Size - key in 200 px X 200 px.
Click with the marquee tool anywhere in your image, and drag this around the screen as with a normal marquee.
Use Image -> Crop to complete the process.
