I've been using a 430EX tethered to my 30D with a sync cord. I know how the flash works off-camera in manual mode, but how does it behave when I fire it off-camera with the flash set to ETTL mode? Does the output of the flash vary or is it constant?

I have a 430ex and have extended my off shoe ETTL cord 25 feet. The flash output is consistent so far for me as I have only used it 2 times, maybe about 75 shots total...
Not to hijack your thread but to add to it I have another question.
When using a 430ex tethered off camera and shooting through an umbrella, does the ETTL adjust from the camera (measures the flash output on the subject as seen throught the camera lens and correctly expose the subject) or from the 430ex (reads the bounce from the umbrella on the speedlite and tricks ETTL into thinking the bounce is from the subject)?

It depends on how the flash is tethered to the camera. If via a PC sync cord, the flash wll fire at full power. If an extended off-shoe cord 2 or 3 cable is used, the output will vary depending on what the camera decides is needed.

ETTL is controlled by the camera based on what the camera "sees" so the exposure is always set from the perspective of the camera, not the flash.

Thanks for your input. . I was taking outdoor portrait pictures with the sun behind the subject and using fill-flash to remove the shadows from their face. My plan was to underexpose the ambient light by 1 stop and light the subject with a bare 430EX on manual at 1/8 or 1/16 using a sync cord to fire the flash. I accidently forgot to put the flash on manual and had it set to ETTL for the first few frames. The pictures came out nicely, much to my surprise - thus my original question. I thought that maybe the flash was varying its output, even though it was on a sync cord, because the subject was not overexposed.

Yes, but you'll need a hotshoe (female) to PC socket adapter so that you can attach the 430EX to the camera via a PC sync cord.

Correct. I fire 2 430EXs with a sync cord (and a splitter). Lon at FlashZebra has all the pieces parts.
