Print release form

anyone have a print release form (to be included with cd sales) that I could take a look at, I am in the middle of writing one, and wanted to see what others others are including on their release!

Mine's a simple insert that states "so and so has permission to reproduce the contents of this disk as they see fit" or some such thing and my contact information with list of recommended online labs. Has my logo up top to look more official.

No big deal really.
