Model release on back of cards?

I'm doing a big event/concert and was thinking about putting a model release of some sort on the back of all photos. Something similar to when you get your photo taken at theme parks or family fun parks.

Do any of you do that? What should be on there?

I just want to be able to use the photos in my portfolio, site, business cards, etc.

Unless people sign it you have no model release.
