I use a couple of 5Ds and have a 580EX on one and a 580EXII on the other. There have been some random times when the flash completely overexposes the scene. This is true even if I have it in manual mode and shoot one image perfectly and the next image with the same camera and flash settings. I don't know why this is happening and it doesn't seem to matter if I change around the flashes on the different bodies.
The really weird thing is, when I look at the overexposed images in Photoshop and look at all the EXIF data, it says the flash didn't even fire. I can clearly see flash shadows in these overexposed images, so I know it's not the camera that's overexposing, but I can't explain why they a) randomly overexpose and b) why my exif data on the CF card and the info from Photoshop would both tell me the flash didn't even fire at all.

Could it be E-TTL communication screw up? I'm not sure whether proper flash info shows up in the EXIFs if only the trigger pin is activated. I assume you cleaned the pins, but could they be pushed in a little?

Have you tried cleaning your hot shoe contacts on the camera?

Maybe some schmutz is causing a communication error?
