I've been searching high and low.. I haven't found anyone selling one..
Does anyone know if there any companies that make this?

maybe just cut out one of those el cheapo foam "picnic plates" from somewhere like WalMart. You could then play around with adding some stand-offs to create some distance between the flash and the plate being used as the diffuser. I suppose even a sheet or micro foam cut to fit would work but my first thought is a ring cut out from a plate adding some spacers to create that distance from the front of the flash and the elements inside the flash.
Would cost you all of a couple bucks for like 25 plates and you could make spacers from the left over bits of plate...or you could get fancy and use bowls made of the same foam and cut out a hole in the middle for the opening. Nice thing about the bowls is you could attach them easier as they are already close to the lens body and probably would require less bending to shape and fit. I would secure them with painters tape which leaves no residue as long as you don't leave it in place for weeks or months at a time. ;)
BTW, I answered this even though it's a week old because I am considering this ring flash over the Canon option.

It's hard to understand why no such thing is sold commercially. Studio portrait photogs love ring flash, but there's no E-TTL capable portrait ring flash except for this really expensive ring flash adaptor which only works with the 580EX (and Sigmas after some modifications) ...

Because all these on-camera ring-flashes, including the expensive adapted ones, are far too small to be any real use in the studio. They're scarcely bigger than the bare bulb and backplate of a studio strobe.
Oh, and not all of us love ring flash :)
