Here's what working:

It power's on
All the dials and function/button seems to work
New battery installed
When mounted on the camera, auto focus assit beam works with camera is in AF.

Here's what's not doing:
Pilot light is not green or red
Even when I press the pilot button, no flash.
When shooting, no flash when mounted on the camera.

If the light is neither green nor red it's a battery issue. Buy some new alkaline batteries to prove it's that, then replace your rechargables.

Well, I wish that was the case. The rechargeable batteries are brand new and they work perfectly fine on my other 580EX II.

Reset all of the custom functions to "0" and try again.

I would try a new set anyways.

It's broken, send it to Canon for repair.
