A flash (580EX or Sunpak 120j or SB-24), flash bracket and a softbox (http://www.adorama.com/WEARF.html?searchinfo=westcott%2028&item_no=1)

I'll need a clamp as well.

This is the stand:

Can someone recommend a boom (and clamp) for a home studio (8' high, 12' wide, 40' deep)? It doesn't have to be super nice. Functionality is key.

Thanks!

How deep is that SB? Are you looking to hang it over someone?

Not directly over someone (unless they were close to the ground).

I need it up high and angled downwards if possible. With the way the Westcott Apollo softbox is designed the flash is inside the softbox so there isn't an ideal way to bend the box up and down.

I recently purchased an Opus mini-book kit (boom & stand). http://www.opusprophoto.com/products/accessories/general2.html

I would most likely hold a hot-shoe unit and small softbox like that. I use it to hold up a B400 but it it's not the sturdiest set up.
