That one looks pretty easy though quite time consuming. I assume you are using Photoshop.

Add a plain white background layer. All three shots against a white background and brought into Photshop on seperate payers. The two shots in the background are on layers below the pic where she is laid out and have their layer opacity reduced so that the white background shows through. In the main pic she has been extracted from the background using a layer mask or using the CS extract tool. This is the time consuming bit as hair is quite difficult to extract. The CS extract filter does a really good job but takes a bit of practice. If shot against a white backdrop, adding a white background back in helps. Extracting and then adding a dark background is more difficult as the odd stray white pixel left over really shows through.
