Flash bracket for long lenses (red-eye avoidence)

When using the 580ex and better beamer, I often get red-eye on my bird subjects. I'm wondering how far off the lens the flash should be and for recommendations for a bracket. Would greatly appreciate any help.

I use the Wimberely Telephoto bracket . And I just purchased the M-6 extension post to make it 5 inches higher.
