I have a Velbonsliding macro plate which is very similar. I like it when I need very precise placement but it's usually too cumbersome for my patience level and I generally just hand-hold and focus by moving my body/camera/hands.
That is, if I really need ultra precise placement for a macro shot and it can't be hand-held then I usually skip the shot. :-)
If you have patience to set the thing up and use it then these things are extremely handy, and I would easily recommend one. I really have very little patience.

I have the 454. It's extremely well built and very easy to operate. I bought it to go with my ML-3 and 100 2.8 Macro when I was in New York last December but on 29th December was hospitalised with leg fractures and haven't had chance to use it 'in the field'. But now spring is here and I'm well on the road to recovery I do intend to try it out soonest.
As Scottes says you do need the patience to set it all up. I haven't had any of the reported 'backlash' on my in-house trials with the 454 so I'm looking forward to using it under fire.
