What kind of light setup is needed to make such an effect? How many "lights" and how to use them?

Now I am no expert but the finished product look almost the same .

It looks a lot like a ring flash. Bright hot spots in the center, even shadow under the chin.

Not really. You could duplicate the small light source, but you would probably not be able to get the same wrapping effect.
Also, a diffuser probably would do too much to help in this situation.

From the general look, I'm guessing a beauty dish up high, and maybe something soft and low for fill.

From what I could make our from the "behind the scene" video of that SI shoot, it was an ordinary but large softbox placed behind the photographer. :)

Frankly, I wouldn't care what kind of lighting as long as I can have an opportunity to shoot such a subject... :)
