Any thoughts? I do not like the perspective, the left and bottom appear straight but the right and top are not. Any way to fix this in PS_CS? Thanks.

How about this?

In PS use the Transform - Distort tool to drag the sides until they are horizontal / vertical.

Thanks. I tried that and saw "jaggies" on the top and sides but noticed once accepted they converted fine.
