I'm curious whether this combo will cause vignetting on either 17-55 IS or tokina 11-16. Anyone know?

Probably...you talking about ~3 filters as the CP is about 2.

You are asking questions that are totally dependent on the specific design of a given lens. You will need to try out on each type of lens with the specific "solution". I could give examples...but you know what I mean.
Whatever path you take (and are trying to do) may result in vignetting at the wide settings. What you may need to do is use a less wide angle...step back a bit...voila, no vignetting... :-)

Well...yeah...but...
...how would you suggest one get a deep ND effect with polarization?
Sometimes you just need to do the "wrong" things to get the effect you need to get.
In this case, to avoid problems, I would suggest using a hood to reduce stray light. I would also mount the CP first, then the ND on top of it. The ND will dramatically reduce the light transmitted, which should also reduce the chance of stray reflections off of the add'l glass surfaces.

Probably worse, as the holder and filter are thicker than a typical CPL. Plus, it would be difficult to adjust the CPL (unless you have very small fingers).
But to answer another question...you can stack filters, as long as they are of very good optical quality (after all, the Singh-Ray Vari-N-Duo is nothing more than a stack of an ND filter and a warming polarizer). On a full frame camera, this will vignette at 24mm and wider. You most probably may get some vignetting on the 11-16, as you had asked. But as someone else has stated, it all depends on the lens and camera type. The only way to know for sure is to just try it out. Another option is to handhold the Cokin P-filter in front of the CPL.

OK, I'll be using it with a 40D. I'm interested in getting an ND filter and I thought that there might be situations where I would want to use it with my CPL, so I'm basically trying to figure out what the best type of ND filter (circular or rectangular) would be for me.

Vignetting isnt the monster its made out to be either. You can live with it if you need a certain effect you will have to make sacrifices.
