Some version of 055 tripod with this (http://www.bogenimaging.us/Jahia/site/bius/pid/6780?kindOfProductCollectionRequest=productDetail&productCode=3152K&productDescription=VIDEO%20MONITOR%20HOLDER%20KIT&curBrandId=MAN&market=MKT1) holding the laptop it looks like. His black shirt makes it a little hard to tell what's on the other end of the center column;

That actually does not look like a manfrotto tripod to me...it looks like it has twist locks and hinge joints/mounting head more like gitzo/benro/etc.
The ballhead looks like the RRS BH-55 LR (lever release):

he used to use slik and now uses gitzo, as i recall. and i believe gitzo makes a laptop platform.

I couldn't figure out what he use for the head either. :) If i don't rememer wrong, the center colum he uses belong to Gitzo also. I remember saw it in Gitzo catalog.

That is a Manfrotto item between the tripod and the computer and ballhead

As I said, I'm not sure what it is, but Manfrotto does exatcly what you're looking for. They call it "Q90°" which the center column rotates into vertical/horizontal position. I'm using the horizontal mode for Macro photography.
There were few tips for the laptop tray which you can also get from manfrotto.

...but that solution lacks attachment for the camera (head) as well as the laptop (or a second head with camera)

Well, from the photo both the laptop/monitor platform and the head at the other end are clamped onto the center column.

Nope! I own what is pictured..the entire horizontal piece is a single product, and the 'clamp' which you see is included...(close up of one of the ends...)
