model release

im looking for a model release agreement form. yes i already googled it lol. i came up with a few. what do you guys think of these? any others i should look into?

lets say im doing time for * work. i want to include a "due date" for the photos to be edited. how should i add this?

The istockphoto looks good, but the NYIP release is too basic. I always have models I work with sign a release, even if it's a trade shoot. I never give an actual date for finished images, usually just say give me a week or so. If you google "model release form, you should get plenty of places to start from.
