Tripod?
Video + Photography on two separate cameras shooting at the same time?
Some sort of accessory T-shaped accessory on your tripod with ballheads mounted on both sides?

Um, why anyone would want to do that beats me. The video will pick up sound from your dslr usage (shutter, changing of settings, autofocus, IS). Seems like it's mounted via a bracket that's meant for a flash unit.

So why not get a 5D mark 2 with dedicated HD video and retain the ability to shoot full 21mp images with a 1 sec pause in between the video?
I don't see the point in having such a setup. And it's not just the noise that will detract from the video experience. You'll most likely be fiddling with the dslr more than the video camera, making all the jerking very obvious.
But, like I said, you should be looking for a flash bracket.

I've seen videos of paparazzi using flash brackets, store bought or home made to mount a point and shoot camera on top of their SLRs. I presume they do this to capture video, and of course the video quality hardly matters as long as the scene is captured.

Oh, i forgot to add. If you are not using a hotshoe flash, there is a hotshoe-ballhead from Manfrotto called the 482LCD where you can mount your small point and shoot video camera.

Costs about US$50
