I have a lens shade that is about 3-4" and it fits on my 135mm lens. I rarely use it just because . . . well, I don't want to pack it around and the lens looks longer and seems to draw more attention in some casual situations . . . I was wondering though, how much better would my images be? Does the shade help with getting a better exposure, reducing lens flare, all of the above . . . What is the primary purpose of the shade. How often do you folks use your lens shades? Always, sometimes, only in certain situations? Thanks.

Out on a sunny day, it help block light from coming in from the side providing shade for the lens. I think they call it lens hood. I might be wrong!!!

The hood reduces lens flare and increases contrast. Try it yourself in a few situations and decide for yourself. I'd recommend trying it in bright daylight, in near dark conditions with a bright light to one side, and inside, along with any other situations you commonly photograph.
