The manual indicates it can be done; I can't figure out how. Am I just missing something very obvious?

Mine is turned off in all Creative modes, without any special settings. It only works as fill-flash if I push the button to pop up the flash, or put an external flash on it that's turned on. The basic modes the flash pops up by itself when it thinks it needs it.

I have a 20d and was just looking for the same information. I have to take pictures of my daughters concert and I dont want the flash to go off. I'll have to try this tonight.

You have to manually lift the flash to get the flash to fire in any creative mode. Just make sure that the flash is in the down/off position.

By that, you mean that you have to push the button for the flash to pop up.
