I was hoping someone could provide me with a template that they use as a model waiver. I want to do some portraits of children and hope that some of the parents will give me permission to post them publically on my website. I would like to have the parents sign a waiver giving me permission to do this.
Anyone have a waiver they could share with me?

I have one on here (that I feel covers everything!)... not sure exactly what the thread was called though... hit my name, and hit 'find all posts by mhappy'! good luck!
... One thing I ran into though; here (in Canada) if you're intending to use a minor's images for the purpose of advertising (on your website or otherwise)... if the parents are divorced and the mother has the most custody and the father is the one who you're dealing with for the shoot... the mother's signature is the one what needs to sign on the child(ren)'s behalf.
I went through a similar situation this past summer... the mother wouldn't sign the form, so I have these awesome images sitting in a file, and they're no good for anything... as apparently, the father (my brother-in-law) can't legally sign the release for so I can use the girl's photos. ???

great advice on the signature... but if the father of the model is your brother-in-law, wouldn't that make the mother of the model to be your sister?

great advice on the signature... but if the father of the model is your brother-in-law, wouldn't that make the mother of the model to be your sister?

No... the mother of the model is my husband's brother's ex-wife.... (sigh)
