How important is it that your model signs some kind of a form at the shoot if you are not a professional photographer? Is this necessary, or do you look like a tool...? Thanks!

It's important, I have had every single person I have shot from the beginning sign one. I don't think you can ever look TOO professional.. you can look unprofessional.. but it's always better to err on the side of cautious IMO.

Shooting should not commence until the model's ID has been verified (and photocopied or photographed), and a release has been signed.

Model releases are not necessary unless you're using that persons image for commercial usage. Really depends on what you're shooting and if it's realistic to get one of everyone - model shoots obviously you want one on file so you can use the image how you please (your advertising for example).
