Documents needed for portraits

If you are shooting portraits for hire, what documents do you use? Are you having the client still sign a model release or any acknowledgment of ownership of the photo? I am starting to get requests for paid work and want to make sure I am covering my bases. Thanks!

I use a portrait contract that I copied from PPA and made a few modifications. I also (sometimes) use a model release but that is not as necessary. All I ever do is post them on my blog or portfolio.
