Wedding album order form template

Anybody know where I can find a wedding album order form template?

I have searched google and found nothing. Just need a general layout to create my own.

Wow. I've tried to make one, and can't do it. There are simply a billion options out there.

Your best bet is to work with your client to narrow down which type of album you want -- Graphi, KISS, pick your favorite. Then, based on that manufacturer, use their ordering process to get it right. Graphi you can pick a page orientation (tall, wide, square), and a multitude of sizes. KISS, area all square, and only a couple sizes. You get my point...

But, in short, I think you need to follow along the manufacturer's ordering process, and back-engineer that into a form that's friendly to you, (or 2 or 3 forms, based on how many manufacturers you sell).

What do you want/need an order form template for? I have the basic album type specified in their contract, then I have an album confirmation document I mail/email to them when the design's done.
