Need CPS equipment submission form

Hi.
I need to send in equipment for repair, but I'm out of forms. I have contacted CPS for more blank ones, and you'd think they would be happy to send me more since each repair makes them money, but to no avail. If anyone has a blank form they can fax or e-mail me, I would appreciate it very very much, as would my sickly lens.

Yes, there are several. But, you would need labels too. I'll scan them at work tomorrow as my home scanner is offline and I'm too lazy to hook it up. They will be in B&W. Seems weird no response from CPS though as I have (so far) had great response from them.
