I did a outdoor shoot with a lady and her husband's motorcycle club's bikes. How would I go about getting a model release? Do I just make her sign my standard release like what I do for TFCD? How does it work for paid shoots?

I'm not quite sure what you're asking, but if you're talking about the bikes you need a property release from each owner to publish pictures of the bikes for commercial use. If you are talking about people then a model release would be needed. If the usage is for the people or bikes in your images then no release would be required unless you see possible commercial use for the images in the future.
I use the releases at ASMP.org, they can be modified to your particular purpose.

I am talking usage for my website and blog. I do not need model release to display the pictures on my personal website and my blog?
Do I only need model release when I intend on selling the picture, print, commercial use etc?

It's always best to have a release if you can get one, if you use the website to advertise your services that might be considered commercial usage. Most people wouldn't consider it needed to publish pictures here for instance or on a site like Flickr. You absolutely do need a release if you intend to sell the picture for commercial use. It wouldn't be needed to sell the prints as art, but it would be a good idea to get one if you can.

I’m working on the launch of Stockphotorights.com, a new educational resource which aims to unravel the complexities and expose the potential legal pitfalls of image use, and thought the site would be of interest to the people here. Stockphotorights.com has been set up by Getty Images with the support of BAPLA and PACA and provides a simple, user-friendly, one-stop shop for anyone wishing to learn more about licensing stock imagery for commercial use.

Here's a question about releases.
I took some pictures at Church and made up some composite prints for some people there. One of them I really like how it turned out and I thought about adding it to my portfolio just to show what I have created. Do I need a release for this, if its just in a book thats laying in a barber shop for people to browse through? Or to even display in my studio? *my studio is in the same building as the hair stylists so my work is displayed in there :D*
I know the easy answer is to just get releases from the people in the composite, but one of them I dont see often, the other one I see more often but not that often and the main subject I see pretty often, but their all under 18. The composite is a nice decorative background, and i took a picture of the 3 subjects and converted it into a sketch in photoshop, and played with blending modes and masks in photoshop to blend the sketch into the background.
