Single diffused Yongnuo 568 flash, camera triggered and monitored by DSLR controller over wifi. Not at all complicated but I am relatively happy with the result.

No I don't, but they are 4"x4" mirrors from a craft store. I used them in lieu of adding additional lights as their direct reflection offers a directional light just as bright as the light hitting them. ;)
