Yellow fever vaccine is only for those traveling to certain parts of Peru, so if that's where you're heading then its also good to have. Rabies vaccine is only necessary if you are at risk (e.g., veterinarians, bat researchers, etc.).

I recently returned from 3 months Ecuador / Peru. Ecuador was mostly doing field work, whereas Peru was solely for travelling... but photography/sightseeing wasn't the main focus of my summer so I had to pack light with the camera gear.
I brought a T2i with a Canon 15-85 IS USM lens and a spare battery and that worked out really well for me. That 1 lens was pefect - but I was mainly shooting landscapes, urban environments, and occasionally some people shots (when it was acceptable). For the city and for people, I wish I had brought my 50m f/1.8 or my 85 f/1.8, but it didn't make sense as I was moving around constantly. If you're photographing animals, I can't help you there. My personal reccomendation is a wide angle with decent range and IS (15-85 is a little heavy but it was okay). Especially if you're hiking around at high altitude (Macchu Pichu wasn't too bad, Lake Titicaca reason the altitude was really noticable at 12,000+ feet.) My other biggest advice: use your camera whenever you can. I used mine a lot in more rural areas, but I shied away from using it a lot in bigger cities for free of crime/theft (Lima, Quito, etc) and there are a lot of shots I missed out on because of it.

a wide angle and an all around lens would serve well. do take portrait lens too if you wana capture the lifestyle there
